Instruction on Filling in the Affidavit

 

  1. DO NOT sign until you are in front of a commissioner of oaths (see below).
  2. Read the affidavit of Innocent Gwisai and decide if you agree with it in so far as it concerns you. That will mainly be that these abuses talked about in this confirming affidavit are occurring and also the values of the house, bond amount and the amount the sheriff sold it for.
  3. Fill in your name at the top where it says full name.
  4. Fill in the name of the bank you had problems with.
  5. If there were two banks involved, fill in two forms.
  6. Read through the affidavit and tick what applies to yourself.
  7. If you tick anything between 19 and 32, then you should consider ticking 33 as well.
  8. Most people should tick one of the options between 35 and 37.
  9. Always tell the truth completely, do not exaggerate at all.
  10. You can add additional details in the box at the bottom (38). Write legibly and keep it relevant to the case. Additional details may be the dates when events occurred making references to the boxes you have ticked stipulate the details of what occurred.
