Instructions on how to fill out an Affidavit at your police station

 

HOW TO FILL OUT AN AFFIDAVIT AT THE POLICE STATION

 

POLICE STATION AFFIDAVIT

 

1. Go to your nearest Police Station
2. Go to the Community Service Centre (CSC) also known as charge office
3. Make sure you have your Identity document with you (ID BOOK)
4. Tell the Police Officer you would like to make an affidavit
5. The Police officer will give you a form
6. Fill out all the fields required
7. The Police Officer will stamp that affidavit
8. Process complete

 

YOUR OWN AFFIDAVIT

 

1. Go to your nearest Police Station
2. Go to the Community Service Centre (CSC) also known as charge office
3. Make sure you have your Identity document with you (ID BOOK) and your affidavit
4. Tell the Police Officer that you need to get your affidavit commissioned
5. In front of the Police Officer initial each page and sign the last page where it says Deponent
6. The Police Officer must then initial each page and commission the last page.
7. Process complete
