When you sign this you are agreeing to this which is the case against the
banks filed in 2020 and saying that you have read it:
After you have filled it in you need to get it commissioned at the police

then scan it and send it back to completed@drdouglasjshaw.co.za
The Banking Law Advisor team dont charge you anything to do this (the
class action is on a contingency fee basis which means that the legal team
may receive 25% of the final proceeds up to a maximum of double normal
fees- which means they have been working for nothing on this for a long
time, because they believe in it.
This means that they do not have the resources to respond to mails and
calls from people who might benefit from the class action (there are an
estimated 100,000 people). What we can do is add you to the class action
mailing list so you get updates.
